Cub Scout Pack 200 Permission Slip
As the parents or legal guardian of ____________________________, I hereby give my
permission for him to participate in Pack 200 activities as noted below.
I give permission to the leaders of Pack 200 to render First Aid, should the need
arise. In the event of an emergency, I also give permission to the physician, selected
by the adult leader in charge, to hospitalize, secure proper anesthesia, order injection
or secure other medical treatment, as needed. I further agree to hold the above named
unit and its leaders blameless and I release them from liability for any accidents that
might occur during Pack Activities, except for clear acts of recklessness or wanton
disregard of Boy Scouts of America policies and guidelines.

During the course of normal scouting events occasions may arise when mass media organizations,
such as newspapers or television stations, provide coverage of activities. Scouts are often
selected to be photographed, filmed or interviewed as part of such news releases. Pack 200
regularly publishes the names of boys and family members that have participated in local pack
activities and publishes the recipients of badges, pins and awards at monthly pack meetings,
in local newspapers. I give permission for my son and family members names to be included in
such press releases.
